Let’s first take a look at how the drug works.
Generally speaking, modafinil causes levels of histamine in the brain to rise, causing extreme alertness in the user  and preventing feelings of sleepiness or fatigue . Modafinil also enhances working and episodic memory  by increasing the amounts of dopamine, serotonin and norepinephrine in the brain .
A lesser-known mechanism of Modafinil is that it reduces reactivity to fear stimuli in the amygdala, the region of the brain that’s central to the modulation of anxiety . This mechanism lets the user think more clearly and work more effectively, even if they are stressed or are under a lot of pressure.
This provides great context for going into the cognitive benefits of modafinil.
In fact, research shows that modafinil has a significant effect on “mood, fatigue, and cognitive performance” in the course of sustained “sustained mental work” . As you can see, no bad outcomes — even at the mind-bogglingly high dosages administered.
That said, there have been unconfirmed reports of fatal overdoses involving modafinil alone. But we would question the veracity of such reports, since we could not fathom the quantity actually ingested. After all, the “LD50” of Modafinil (i.e., the dose required to kill 50% of test subjects) in rodents via oral administration has been found to be 1-1.25 g/kg . Assuming that LD50 is the same for humans, a 155lb male would need to stomach 75 grams of modafinil to attain the so-called “lethal dose”!
To our knowledge, nobody has ever been confirmed to ingest 12 grams of modafinil in a single day.
The lesson: We see very little to be concerned about as far as modafinil overdose. In any case, users are well-advised to be smart and stick to their minimum effective modafinil dose.
Modafinil users should be fully aware of all potential interactions with other medications, specifically CYP3A4/5 substrates (e.g., steroidal contraceptives) and CYP2C19 substrates (e.g., phenytoin, diazepam and omeprazole).
This is because modafinil simultaneously induces and inhibits certain metabolic enzymes in the liver involved in the processing of these drugs :
“The mean activities of microsomal CYP1A2, CYP2B6, and CYP3A4/5 from modafinil-treated hepatocytes were higher (up to 2-fold) than those in the solvent-treated controls but were less than those produced by reference inducers of these enzymes.
At high concentrations of modafinil (>/=100 microM), the mean activity of CYP2C9 was decreased (up to 60%) relative to that in the solvent controls.”
Moreover, modafinil is known to decrease the effectiveness of certain types of birth control and thereby increases the likelihood of pregnancy in patients otherwise taking birth control .
We would finally add that care be taken if taking blood pressure pills with modafinil, as the drug can counteract the activity of anti-hypertensive drugs. It is also prudent to exercise caution when mixing modafinil with caffeine (or any other stimulant), as the synergistic effect can overwhelm many users.
